Many people with Parkinson’s are dissatisfied with the care they receive in hospital. Management of their Parkinson's Disease sometimes suffers while other medical problems are being addressed. Hospital staff may not be familiar with common complications of PD nor realise the importance of giving medications on time. Previous research has shown that patients suffering heart attacks do better on coronary care units than on general wards and stroke victims have improved survival following care on specialist stroke units. Previous research has also shown than a specialist multidisciplinary Parkinson’s rehabilitation unit can improve walking and self-care abilities. This study will compare the experience of people with Parkinson’s who need urgent admission to hospital before and after the introduction of an SPDU. We will study the effect of the SPDU on drug errors, patient satisfaction and length of stay, and explore the feasibility of a definitive (randomised) trial to see if the results could be broadly adopted.

Patients with the following conditions will not be included in the study: • Vascular Parkinsonism • Lewy Body Dementia. • Multiple system atrophy • Progressive supranuclear palsy • Drug–induced parkinsonism • Primary diagnosis requiring placement on other specialist unit: hip fracture, myocardial infarction, unstable angina, acute stroke, gastrointestinal bleeding, any condition likely to need urgent surgical intervention, acute renal failure requiring admission to renal unit, shock or multiorgan failure requiring ITU, respiratory failure or other chest disease requiring specialist care on respiratory ward. We estimate that 30-50% of urgent admissions will need to go to alternative specialist units. Different interventions are required for these patients and should be the subject of a separate research project.
